Submit an application when our waiting list is open

Due to high demand, our waiting list may not always be open, during it's open period, you can submit an application, this will include household information, income, and a few other questions so we can determine if you qualify for a subsidized program.

To apply you'll need SSN for all family members, birth certificates for all family members, income, deductions, and assets. You'll also need to be ready to answer a series of questions as truthfully as possible.
